Skilled Nursing

The registered nurse plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. Some essential job functions include the following:

 

  • Completing an initial assessment of patient and family to determine home care needs
  • Providing a complete physical assessment and history of current and previous illnesses
  • Initiating the plan of care and making necessary revisions as patient status and needs change
  • Developing a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
  • Administering medications and treatments as prescribed by the physician
  • Communicating with the physician regarding the patient’s needs and reporting any changes in the patient’s conditions
  • Detaining and receiving physician’s orders as needed
  • Participating in on-call duties

Certified Nurse Aide (CNA)

A CNA works under the supervision of a nurse or therapist and performs various services to help meet the patient’s personal needs, promote the patient’s comfort, and a safe environment. Some responsibilities include the following:
 
  • Providing or assisting in personal care
  • Bathing, grooming & oral hygiene
  • Assisting with dressing
  • Assisting with toileting activities
  • Light housekeeping & light meal preparation
  • Assisting n transfer and ambulation as needed

Physical Therapist

The physical therapist employed through the organization is responsible for providing services according to a written plan of care. These services may include, but are not limited to the following:

 

  • Participating and evaluating the therapeutics/rehabilitative/functional status of the patient, and in developing the total plan of care.
  • Evaluating the home environment for safety
  • Initiating PT program and instructs other personnel and/or family/caregiver members in recommended home exercises.
  • Participating in case conferences as required

Occupational Therapist

The occupational therapist is responsible for implementing a plan to assist the patient in regaining and/or maintaining activities of daily living and basic functional skilled maintenance. Some essential job functions include the following: 

 

  • Evaluating the patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity)
  • Evaluating the home environment for hazards or barrier to more independent living
  • Developing a treatment program and establishing goals for improved function.

Speech-Language Pathologist

The speech-language pathologist is responsible for implementation of standards in regards to symptoms control and/or enabling the patient to maintain activities of daily living. Some essential job functions include the following:

 

  • Conducting appropriate evaluations, assessing the home environment, and identifying equipment needs relative to speech/language function
  • Identifying functional speech defects and establishing a plan of care to improve a patient’s function
  • Administering speech therapy program utilizing specialized therapeutic technique and/or equipment; recommending communication devices/aids as indicated
  • Participating in case conferences as required.

Medical Social Worker

The medical social worker is responsible for providing assistance with social issues for patient and family. Some essential job functions include the following:

 

  • Assessing the psychosocial status of patients and communicating those findings to the nurse
  • Serving as a liaison between patients or families/caregivers and community agencies
  • Assisting the physician and other team members in understanding significant social and emotional factors related to health problems
  • Participating in case conferences as required.
